A notebook for clearer care conversations
Keep questions and follow-up together when supporting someone in care.

Before a care conversation, ask your loved one what they want discussed and how they want to be involved. Keep their preferences at the center.
Use a simple notebook for meeting dates, the names of people you spoke with, questions, and agreed follow-up. Share sensitive records only through appropriate private channels with authorized people.
A simple starting point
- Bring a short list of questions.
- Ask who will follow up and when.
- Write down the next check-in date.
General educational ideas to help you begin a conversation. For decisions about health, legal rights, or finances, speak with an appropriately qualified professional.
Pass a little peace along.
